Microform Retrieval Form

All microforms are stored in the basement of the W.E.B. Du Bois Library and must be retrieved by library staff. Microforms will be retrieved daily during Media service desk hours of operation. 

Please complete the Microform Retrieval Form as completely as possible to request microforms listed in the UMass Amherst catalog. We strongly encourage you to submit your requests in advance, so the materials will be available when you arrive.

You will receive an email confirmation when your materials are available for pick-up. Retrieved Microforms can be checked out at the Circulation/Reserves desk on the Lower Level of the W.E.B. Du Bois Library during all hours that building is open.  Microforms viewing and scanning equipment is located nearby in Room 49 in the Learning Commons.
